Universitas `Aisyiyah (UNISA) Yogyakarta has won the second best award in the Education category at the Paritrana Awards 2023. The award was symbolically handed over by KGPAA Paku Alam X, Deputy Governor of Yogyakarta, to Dr. Yuli Isnaeni, S.Kp., M.Kep., Sp.Kom, Vice Rector II of UNISA Yogyakarta, in an event held at the Kepatihan Ward of the Yogyakarta Government, Tuesday (25/06).

Paritrana Awards is an award given by BPJS Ketenagakerjaan to local governments, businesses, and educational institutions that show high commitment in the implementation of employment social security programs. This award aims to encourage increased awareness and compliance with social protection for workers in Indonesia.

In his speech, Deputy Governor of Yogyakarta KGPAA Paku Alam X congratulated UNISA Yogyakarta.

“Congratulations, hopefully the award can increase awareness and compliance with labor social security protection,” he said.

Yuli expressed her gratitude and pride for the award in the Employment Social Security Category for Education Sector Business Entities.

“This award is clear evidence of our commitment as a university that has been accredited as excellent, in ensuring the welfare and protection of all employees of Universitas `Aisyiyah Yogyakarta,” said Yuli.

She also added that the award includes social security employment facilities for students who carry out field work practices or internships off campus. “We hope that all employees and students who carry out practical work, feel comfortable and protected so that they can improve their performance and achievements. We thank all those who have supported and worked hard to achieve this achievement.”

With this award, UNISA Yogyakarta is expected to continue to increase its contribution in creating a safe and secure working and learning environment, for the welfare of the entire academic community.

The puerperium is a period after the placenta is born and also ends when various kinds of obstetric devices have returned to the state before experiencing pregnancy. This stage of health recovery in the era or puerperium requires care from a whole field and is a period that each of the women goes through after giving birth. During this period a complication of labor can occur either in a direct or indirect way. The puerperium will also last from the time the placenta is born until 6 weeks after birth or for 42 days after the birth period takes place.

In Indonesia according to the World Health Organizations based on data from the World Health Organization (WHO) Maternal Mortality Rate (MMR) in the country of Indonesia has reached an amount of 228/100,000 of live births, this figure is still very much higher when compared to countries that reach the amount of Vietnam 59/100,000, as well as the country of China with an amount of 37/100,000. Such matters place the Indonesian state to be one of the countries with the highest MMR, then for the scope of Asia, the Indonesian state is at the highest rank number 3 and one of them is infection with a percentage of 50%. According to the Central Bureau of Statistics, the mortality rate in Indonesia has reached 189/100,000 live births, and the live birth rate in the Special Region of Yogyakarta has reached 58/100,000 live births.

The cause of most maternal deaths is the presence of a puerperium infection whose origin is from the birth canal opening which is a good location for the growth of germs. The discovery of perineal wound problems experienced by 75% of mothers giving birth vaginally in 2013 found that out of a total of 1951 spontaneous vaginal births, there were 57% of mothers who received perineal sutures with 28% due to episiotomy and 29% due to spontaneous tears. This could be due to the mother’s low postpartum immune system, poor perineal care and hygiene.

Efforts that can be made to prevent the risk of infection are to do perineal wound care, wound care is done in a good way so that it can make wound healing so perineums and so much faster, the role of the family in the healing period of postpartum women by providing psychological support, watching the mother’s psychological patterns, food intake, rest patterns and can also be from the fulfillment of Vitamin A, because Vitamin A can provide assistance for the inflammatory process that occurs during healing. Vitamin A can be obtained from oranges, carrots, potatoes, and eggs.

The ignorance of postpartum mothers about good perineal care and also correct can make the occurrence of infection in the perineal wound area, the possibility of infection will be much greater for mothers who have less or low knowledge due to errors that exist in the care of perineal wounds and also whether the postpartum mother has applied a high-protein diet yet because the fulfillment of her protein needs is increasing to provide assistance in healing wounds both those in the uterine wall or those in the birth canal wound that experienced a suture action, The protein is needed to be used as a building substance that builds the body’s muscle tissue and accelerates the recovery of wounds that return, and also early mobilization is not applied during the postpartum mother because if applied it can make various abdominal and pelvic muscles return to normal so that they become strong and reduce pain, better bowel and bladder function and speed up the various organs of the body back like everything and make blood circulation smooth or normal.

Midwives as an educator in their practice provide health education to postpartum mothers and families by using leaflets or direct counseling media. Leaflets are used because they have the advantage of providing information that is much more detailed and detailed as well as easy to carry anywhere. It is hoped that after the provision of health education conducted with leaflets and counseling, the knowledge of postpartum mothers and their families will also increase. With this increased knowledge, it is hoped that it can contribute to efforts in preventing infections or complications to reduce maternal mortality and also improve health status for families.

Subjects that need to be considered by mothers during the postpartum period:

  • Conduct visits and control actions with a minimum time of 4 times, namely at 6 hours, 6 days, 2 weeks, and also 6 weeks after delivery.

The purpose of the importance of conducting postpartum controls / visits is that by visiting up to 40 days of the postpartum period, midwives can monitor the various changes that occur to postpartum women such as the involution stage, monitoring the amount of bleeding and also the lactation stage.

  • Checking blood pressure, perineal condition, vaginal bleeding, uterine contractions, signs of infection, temperature in a routine manner, and fundus height.
  • Assess gastrointestinal function, urinary function, headache, wound healing.
  • Ensure that the mother’s psychological state is still in good condition. How is the atmosphere of emotion that he has, make sure also get a support from the family, as well as from the partner. Because psychological support for postpartum mothers can prevent depression and anxiety.
  • Provide education on danger signs in postpartum women such as excessive bleeding, fever, smelly vaginal secretions, shortness of breath or fatigue, severe abdominal pain, swelling in the face and limbs, and swollen and painful breasts. Due to the knowledge of the family as well as the puerpera about the various kinds of danger signs of puerpera, it is very necessary so that the puerpera and family can immediately come to the health facility if they are experiencing danger signs during the puerpera period as described above.
  • Provide education about Vulva hygiene because the postpartum period causes complications, including causing infection in the suture wound or on the skin, thus making the healing process of the suture wound slow so it is necessary to clean the vulva and perineum because it can prevent irritation and provide a comfortable feeling to the postpartum mother.
  • Maintain rest patterns in postpartum mothers who need adequate rest and sleep which is very useful for making breast milk come out smoothly, accelerating the stages of uterine involution, and restoring the situation and conditions after pregnancy and childbirth so that they can carry out daily activities because if there is a lack of rest or sleep in postpartum mothers, it will reduce breast milk supply, slow the process of uterine involution, and also make the mother unable to care for the baby and feel depressed.
  • Provide an education to mothers about balanced nutrition, especially in terms of increasing protein consumption. In theory, balanced nutrition is the composition of daily food that contains nutrients in the type and amount that is adjusted to the conditions and needs of the body, one of which is balanced nutrition to make pereium wound healing faster is protein. Protein is very influential in the stages of healing of perineal wounds because protein has unique uses and functions that cannot be replaced by other nutrients, namely the maintenance of body tissues, growth, and also the repair of a tissue, one of the examples of protein is fish, chicken, nuts, and eggs.
  • Education related to when coitus is safe to do after childbirth and also planning for the next pregnancy and the use of contraceptives, the purpose of this is so that the reproductive organs become much faster to recover and minimize the risk of infection of the uterus. As well as spacing the birth of children, improving the welfare of families, as well as increasing the life expectancy of babies and mothers.
  1. Providing education about the importance of Bounding attachment, namely, not separating the mother and baby during the postpartum period so that the mother and baby are in the same room, always paying attention to the baby’s attachment by the way the mother always takes care of the baby. The benefits of Bounding attachment are that the baby feels cared for, loved, the baby feels safe, strengthens the emotional connection that exists between parents and the baby, helps parents be more sensitive to the needs and signals of the baby.
  2. Educating mothers on the correct way to breastfeed. This is beneficial in preventing nipple chafing, preventing breast swelling. In theory, mothers can understand the importance of proper breastfeeding education, which will build a strong emotional bond between mother and baby, and the baby gets optimal nutritional intake.
  3. Providing education about the importance of taking vitamins because giving vitamin A to postpartum women aims to improve the quality of breast milk, then increase endurance, help postpartum health recovery. The provision of pain relievers in the form of Profen with a dose of 200 grams aims to help reduce the pain that causes discomfort in the postpartum period.

The impact of a lack of education in the puerperium is birth canal infection, oedema, resulting in open wound sutures that can cause bleeding. For this reason, it is very important to conduct a postpartum visit or education about postpartum according to the standards and recommendations that have been set. These various small matters can have a big effect if a good and proper treatment is not immediately carried out.

Stunting due to chronic malnutrition during growth is not only a health problem, but also a serious threat to future generations. In the Special Region of Yogyakarta (DIY), the prevalence of stunting rose from 16.4% to 18% according to the latest BKKBN data for 2024. This suggests the need for concrete steps, including an in-depth evaluation of the measurement methods and accuracy of the tools used.

BKKBN responded to the increase in prevalence by issuing BKKBN Head Circular No. 3 Year 2024, emphasizing the importance of simultaneous intervention in stunting prevention in DIY. The intervention program involves the Health Office, Puskesmas, and ‘Aisyiyah University of Yogyakarta in empowerment to conduct large interventions.

Master of Midwifery students from ‘Aisyiyah University Yogyakarta were directly involved through an empowerment practice program at Puskesmas Umbulharjo I and II to evaluate the obstacles faced by posyandu cadres in implementing simultaneous interventions to prevent stunting. The cadres not only conduct routine weighing, but also collaborate with the puskesmas to empower mothers to understand the importance of nutrition for their children. Through monthly weight measurement and nutrition monitoring, posyandu cadres play a key role in monitoring the growth and development of children under five.

Evaluation of the measurement methods and tools used in posyandu is crucial for accurate data. Mrs. Witriastuti Susani Anggraeni, SE, MM as Head of the BKKBN Family Resilience and Stunting Prevention Working Team emphasized improving the quality of posyandu services, training cadres, and integrating nutrition programs as strategies to reduce stunting. BKKBN DIY has also successfully initiated a program through the submission of DIY special funds by providing 2 eggs a day for families at risk of stunting since 2023 and continued until 2024.

“The cadre mothers, although unemployed, make an extraordinary contribution. They are the spearhead and the tip of the spear. The spear is extraordinary too,” said a cadre.

Increasing the capacity of posyandu cadres on anthropometric measurements, growth monitoring, and toddler food is the main focus in reducing stunting rates. Posyandu cadres serve as the frontline in weighing and measuring toddlers, the main indicator of a child’s stunting status. Training cadres in weight and height measurement enables timely decision-making on stunting. Capacity building of posyandu cadres is a crucial strategy in accelerating stunting reduction in DIY.

In addition to training on anthropometric measurements and growth monitoring of under-fives, posyandu cadres need to be trained in advocacy techniques. This includes effective communication strategies to convey the importance of addressing stunting to the community, parents and local government. The training also covers how to access and utilize resources from government and non-government organizations to support stunting prevention programs in their posyandu.

Good collaboration between the government, health offices, puskesmas, businesses, universities, stunting reduction acceleration teams, communities and the media is key to achieving the national target of reducing stunting towards 2024.

Amidst the increasing elderly population in Indonesia, innovations to empower the elderly are getting more attention. According to the March 2023 Susenas, 11.75% of Indonesia’s population is elderly, with the majority living in urban areas. Responding to the challenges of the quality of life of the elderly, Midwifery Master Students of ‘Aisyiyah University Yogyakarta in the context of Empowerment Practice in Midwifery launched an Innovation program “Sedaya Lansia” (Healthy and Empowered Together with the Elderly) to inspire the health and independence of the elderly. In this activity Prof. Dr. Mufdillah, S.SiT., M.Sc as the person in charge of the empowerment practice of the University of ‘Aisyiyah Yogyakarta. The “Sedaya Lansia” program was implemented through collaboration between students of ‘Aisyiyah University Yogyakarta and the Regional Leadership of ‘Aisyiyah Serang Regency. This activity was held at the residence of the Secretary of the Regional Leadership of ‘Aisyiyah Serang Regency, this program targets elderly people aged 60 years and over with the aim of restoring their enthusiasm for life and making the elderly a valuable asset in sustainable development.

With the motto “Healthy and Empowered Together with the Elderly,” the Sedaya Lansia program includes a range of activities designed to meet the physical and mental needs of the elderly. From health education to creative activities such as ecobricking, the elderly are educated on healthy lifestyles, good nutrition, and the importance of regular health check-ups.

In addition, they were also trained in ecobricking creativity, which not only reduces plastic waste but also provides a useful and fun activity “This is my first time participating in an activity like this. It’s very fun and useful,” said an enthusiastic program participant. The ‘Aisyiyah Regional Leadership Organization of Serang Regency hopes that this program will be sustainable and inspire the empowerment of the elderly across Indonesia.

“We hope to develop this program because it is very useful,” said a representative of the organization, in order to achieve better health and independence for all elderly people.

In the era of globalization marked by the sophistication of information technology, efficient system integration is key in improving the quality of health services. In the Blueprint for Digital Health Transformation Strategy 2024, one of the proposed solutions is the transformation of manual recording systems into digital innovations to improve the efficiency, accuracy, and quality of health services in Indonesia.

Students of the Master of Midwifery at Universitas ‘Aisyiyah (UNISA) Yogyakarta in collaboration with students of the Technology and Information Study Program have formulated an innovation in the health sector through a series of Health Education and Service Information System Practice activities carried out (activity date) for the practice period May 27 – June 21, 2024 at PKU Muhammadyah Gamping Hospital Yogyakarta. This innovation proposes the integration of midwifery care and e-Partograph into the Smart’s Medical Record (SMR) system of PKU Muhammadiyah Gamping Hospital, which aims to improve the quality of midwifery care and strengthen interprofessional collaboration in the hospital.

This proposal stems from the need for a more efficient and effective system in documenting and monitoring maternal and fetal health during labor. Currently, PKU Muhammadiyah Gamping Hospital does not have a feature in the SMR that is specifically designed and developed for obstetric care. So far, midwives in the hospital have been using an EMR that focuses more on nursing care standards, making it less suitable for midwifery-specific needs. Seeing this gap, UNISA Yogyakarta students, with the support of lecturers and health practitioners, designed and proposed a childbirth midwifery care module integrated with e-Partograph.

The proposed integration of midwifery care into the electronic medical record (EMR) system at PKU Muhammadiyah Gamping Hospital includes the development of features specifically designed to meet the needs of midwifery practice and follow midwifery standards. These digital innovations include specific features such as SOAP (Subjective, Objective, Assessment, Plan) assessment, labor progress notes, as well as electronic monitoring of labor through the e-Partograph. The development of the e-Partograph enables electronic recording and monitoring of labor, so that midwives can systematically record and monitor the progress of labor. These features are expected to not only facilitate detailed and accurate recording of midwifery care, but also provide more support to midwives in providing quality midwifery services at PKU Muhammadiyah Gamping Hospital.

This proposal is expected to increase midwives’ work efficiency, speed up the documentation process, and improve data accuracy. In addition, the included e-Partograph feature allows midwives to monitor the progress of labor more systematically and accurately, so as to make more informed and rapid clinical decisions. This not only improves the quality of midwifery care but also supports more effective interprofessional collaboration at PKU Muhammadiyah Gamping Hospital.

This innovation proposed by UNISA students is a strategic step in supporting more modern and quality health services, and opens opportunities for other hospitals to follow suit in utilizing technology to improve the quality of health services. By integrating midwifery care into the EMR, PKU Muhammadiyah Gamping Hospital can utilize technology to provide better, faster, and more efficient services to patients. This initiative also strengthens the autonomy of midwives in providing health services, which ultimately has a positive impact on maternal and infant health.
